if you're logged in as an administrator, then on your instance, go to Settings / Administration / Careers website, and then click on 'add questionnaire'. Once you have one setup, you can add them to you open joborders

Hi,

sounds indeed like a permissions issue. Either file permissions, directory permissions or file ownership. I assume everything's owned by the webserver and you've checked it all out?

Hey Zoomiest - you shoudl never have a WSOD.. Make sure you have an old version of PHP to work alongside your old cats, and same for whatever version of MySQL you have, too

Sorry - sounds like you've been through it. If you're simply trying to export the data you can use phpmyadmin to export the contents of the MySQL database to a SQL file, or to a CSV where you could manipulate it with excel.
