I had created a questionnaire and system was working perfectly.

Suddenly i notice the questionnaire head from the joborder UI is missing and also when i try to add a new job orderi cannot see the questionnaire head nor the drop down

when you say it was working perfectly,

- was this recently, or some time back?
- what's changed - have you upgraded MySQL, PHP, etc? If so what version was it at previously and what version is it at now?

What version of opencats are you running?
